Azygos Vein Lead Implantation For High Defibrillation Thresholds In Implantable Cardioverter Defibrillator Placement

چکیده
Evaluation of defibrillation threshold is a standard of care during implantation of implantable cardioverter defibrillator. High defibrillation thresholds are often encountered and pose a challenge to electrophysiologists to improve the defibrillation threshold. We describe a case series where defibrillation thresholds were improved after implanting a defibrillation lead in the azygos vein.

Azygos Vein Coil: Bailout Strategy for High Defibrillation Thresholds
We describe an unusual case in which an implantable cardioverter-defibrillator failed to defibrillate the patient during testing at implant; rather, an adequate safety margin was achieved only after introduction of a shocking coil into the azygos vein. The azygos vein is an attractive extracardiac target for placement of a shocking coil, as it provides an ideal shock vector. Tools designed for ...
متن کاملElevated Defibrillation Threshold: Are Defibrillation Vectors Important?
The evaluation of the defibrillation threshold (DFT) is common after the implantation of implantable cardioverter defibrillator (ICD) devices. The goal of this standard of care is to achieve successful defibrillation of ventricular fibrillation. In patients with elevated defibrillation thresholds, alternative techniques are required to correct the situation. We describe a case in which an uncom...
متن کاملUse of a Tunneling Technique to Achieve a Lower Defibrillation Threshold during Implantable Cardioverter Defibrillator Implantation via the Right Subclavian Vein
A 56-yr-old man with aborted sudden cardiac death underwent implantable cardioverter defibrillator (ICD) implantation. While the ICD was being implanted, a left subclavian venogram failed to visualize the left subclavian vein, which was attributed to likely prolonged indwelling of the left subclavian sheath for venous access. Accordingly, the right subclavian vein was punctured and the ICD lead...
متن کاملImplantation of a Resynchronization Implantable Cardioverter Defibrillator in a Patient with Persistent Left Superior Vena Cava
Implantation of resynchronization implantable cardioverter defibrillator was performed in a patient with persistent left superior vena cava. A dual coil defibrillation lead was inserted in the right ventricle apex via a small innominate vein. Left ventricular and atrial leads were implanted through persistent left superior vena cava. Left ventricular lead was easily implanted into the postero l...
متن کاملSuperior Vena Cava Electrode
Background A unipolar defibrillation system using a single right ventricular (RV) electrode and the active shell or "CAN" of the implantable cardioverter-defibrillator itself situated in a left infraclavicular pocket has been shown to be as efficient in defibrillation as an epicardial lead system. The purpose of this study was to determine whether defibrillation efficacy can be improved further...
